Health Action believes that a healthy community depends on everyone having a source of reliable and available primary care, that will help prevent illness as well as care for chronic conditions (like diabetes and asthma), and acute illnesses and injuries.
Health Action supports a growing movement to improve primary care practices (offices that provide family medicine, internal medicine, or pediatrics). The goal of these efforts is to make primary care more reliable and easier to access, and for the medical care provided to be centered around each patient. We call this a “patient-centered medical home”, and we believe everyone should have one. We are working with primary care practices in Sonoma County to help support their transformation to become patient-centered medical homes. We hope this will spread throughout Sonoma County, so every resident will be able to have their own patient-centered medical home.
