How To Get Involved

Expanding community engagement in improving the health of Sonoma County residents is a key goal of the council. Interested community members are encouraged to attend Health Action meetings.

In May 2009, the Board of Supervisors accepted Health Action's Priorities for Action (2009-2012): Summary of Project Recommendations , which included recommendations for seven specific projects to achieve Health Action's goals and objectives. In accordance with these project recommendations, Health Action has convened six workgroups to oversee program and fund development for these projects. Implementation will be phased as resources become available.

iWALK Steering Group

iWALK is a countywide walking initiative to engage the community to support a culture of active, healthy living.

Food Access Workgroup (iGROW and Food System Alliance)

iGROW is a countywide initiative to connect people with resources to grow their own food -- in their own yards or planters, with neighbors, or in a community garden. Food System Alliance is an ongoing forum with diverse stakeholders to envision, advocate for, and create a vibrant local food system.

Primary Care Workgroup

The Patient-centered Medical Home (PCMH) Learning Collaborative is a learning community of primary care providers who will work collaboratively to transform their own practices and work to support implementation and dissemination of the PCMH throughout Sonoma County.
